If you know of a child who has been injured or killed by the cords of a window covering, please fill out this quick form and send it into us. PFWBS collects this information so that we can keep track of the accidents, document the product that injured the child, and use this information to work on getting that particular brand of window cords recalled through the US CPSC.

Please take pictures of the product and send them to linda@pfwbs.org with Accident Photos in the subject line. When taking pictures, take a picture of the entire product, a close up of the pull cords, then take a picture of the inside of the head rail which is what mounts the window covering onto the window. Note any labels or markings inside the headrail or on the product.

Do not throw away the product. If you no longer want it in your home, please ship it to us directly. We have a large collection of various window covering products that we use to keep up to date with statistical information. This information is used to help us fight to get cords off the market.
